1. Obtain a Live Scan Request Form

Your healthcare facility or agency will provide you with a valid Live Scan Request Form. Double-check that your name, Social Security Number, and birth date are accurate on the form. We cannot correct personal information during fingerprinting.

2. Find Your Nearest TRACE2 Biometrics Location

We have 3 convenient locations! Scheduling an appointment is encouraged. Walk-ins are always welcome. Find your nearest location and hours of operation here.

3. What to Bring

  • Valid government-issued photo ID: Driver’s license, state ID, passport, etc.
  • Live Scan Request Form: Provided by your healthcare facility/agency
  • Approved form of payment (if required):
    • Facility/agency may have a different fee (check with them)
    • We accept credit/debit cards, company checks, and money orders (no cash or personal checks)

4. We Handle the Rest

Our technicians will capture your fingerprints electronically and securely transmit them to the Illinois State Police by the next day. You’ll receive a fingerprint services receipt with a Transaction Control Number (TCN) upon completion.

5. Results and Registry Update

  • The Illinois State Police will process your fingerprints.
  • Results are sent directly to the IDPH and your healthcare facility/agency.
  • The IDPH will update the Health Care Worker Registry.

Applicant Fingerprinting Process:

  1. Complete Disclosure and Authorization:

Before fingerprinting, employers must collect a signed Disclosure and Authorization form from each applicant.

  1. Enter Applicant Information into IDPH Database:

Once you have the signed form, enter the applicant’s information into the IDPH database (“Applicants” tab).

  1. Generate Live Scan Request Form:

Upon completing data entry, the system generates a unique Live Scan Request Form for each applicant.

  1. Applicant Visits TRACE2 Biometrics:

The applicant brings the Live Scan Request Form and a valid government ID to any TRACE2 Biometrics location for fingerprinting.

Important Notes:

  • Accurate Information: Ensure all applicant information on the Live Scan Request Form is accurate. TRACE2 Biometrics cannot modify this data.
  • Results & Updates:
    • IDPH typically receives results within 48 hours.
    • Applicants won’t receive the results directly.
    • The Illinois State Police will notify IDPH via email and update the applicant’s profile on the IDPH website (
